This is a full-time day position Monday thru Friday 8:00-4:30pm-Must be bilingual
Job Summary:
The CHW uses their expertise about the community they are working with to provide education about chronic disease prevention and treatment.
Essential Functions:
- Engage in community outreach and education about chronic disease screening and health
- Learn and apply clinic policies and procedures to Care teamwork
- Educate community about chronic disease prevention and management
- Engage in relationship-building discussion with patients
- Log enrollments, educational activities, and referrals
- Track and follow-up with patients as part of clinical workflows, supporting team-based care
